Discuss the strengths and weaknesses of DSM-IV TR Essay

DSM-IV TR, which stands for Diagnostic and Statistical Manual of Mental Disorders (4th edition), Text Revision was published by the American Psychiatric Association in 2000 and serves as a guide book for many health professionals to diagnose a patient with a mental disorder. It also helps health professionals to determine what types of treatment could be carried out to help the patient. The latest DSM is widely used, especially in the USA and many European countries.1However, it may not be completely followed by health professionals as they know that there are some weaknesses of the latest version of DSM as well. This essay will discuss the strengths and the weaknesses of the latest DSM and new changes for the DSM-V, which is expected to be published in May, 2013. DSM’s strength would be that it standardizes psychiatric diagnostic categories and criteria2, making the diagnosis of a mental disorder relatively easier than it was in the past. It allows health professionals to diagnose a patient, use the DSM to give them possibly the best treatment and overall, help them to cure the patients if the disorder is curable. DSM also has statistical data such as the prevalence of a certain disease in different genders, age of onset of diseases, etc. This allows health professionals to have a very wide range of knowledge which may be very useful for diagnosis and treatments. Besides, the DSM allows a common language for discussing diagnosis. It provides clear criteria for certain disorders so that every clinician would come up with the same diagnosis. This makes sure that a person is not diagnosed with different types of disorders in different clinics.Thus, treating patients more efficiently. Compared to the older versions of the DSM, the latest DSM also has more subtypes and specifiers which increases the diagnostic specificity. As I mentioned before, this increases the chance of diagnosing a patient with the same disorder in different clinics, assuring that they are diagnosed with the correct disorder as misdiagnosing them could possibly lead to serious damage. For some disorders, such as bipolar disorder, there are severity indicators such as mild, moderate and severe as well. This helps clinician to decide the course of the treatment according to the severity of their disorder. This is one of the strength of DSM as using the same treatment for different severity of the same disorder will not be very appropriate or efficient. However, this severity indicator is absent in some disorders such as manic episodes which is one of the weaknesses of DSM-IV TR. These were some of the strengths of DSM-IV TR. However, we know that another edition of DSM, DSM-V is expected to be published soon. This is obviously because there are some, or many weaknesses in the current DSM with which people are not satisfied. One of the major weakness of the current DSM and probably all the older editions is â€Å"Cultural Bias†. DSMs are mainly published by anglo-Americans and most of the behaviour that is considered as â€Å"normal† in the DSM is actually what is considered normal by the anglo-Americans. That is, some of the behaviour that is considered as abnormal in the DSM might be considered as normal in other cultures. For example, in some cultures, people tend to put a lot of emphasis and values filial piety and due to that, they would behave in a way that might not be considered as normal by the anglo-Americans. Would that classify them as abnormal? What exactly is normal anyways? According to the DSM, it it, I believe, what is considered as normal by the anglo-Americans. This is one of the weaknesses of the DSM which is well-known and I think it is because of this reason that DSM might not be as popular in countries where the culture is much different compared to countries like the USA and many European countries. Another weakness would be that DSM promotes a mechanical approach to mental disorder assessment. The clinicians may focus excessively on the signs and symptoms of mental disorders and they might not put much emphasis on a more in-depth understanding of the clients/patients problems. This problem have improved but it is still a problem caused by the DSM. DSM-IV TR also does not consider patients subjective experience of a disorder. That is, the  approach is not a dimensional approach as there is no first-person report but rather, observations are usually carried out which may neglect the more somatic and psychological processes that underlie the symptoms (Flanagan, Davidson & Strauss, 2007).3 Also, DSM causes most clinicians to be primarily concerned with the signs and symptoms of a disorder rather than the underlying cause by giving a list of certain criteria for diagnosis. Another weakness and debate about the DSM is that it is an unscientific system and it is the opinion of a few powerful psychiatrists. This has raised a lot of questions and have caused people to question the validity and reliability of the diagnostic categories as well. The validity and reliability were especially questioned after the Rosenhan experiment in the 1970s in which it was concluded that the sane could not be distinguished from the insane in psychiatric hospitals. Thus, even though the reliability and validity has improved now when compared to the 1970s, it is still a weakness which I believe could not be entirely fixed. However, validity problems of the diagnostic criteria especially arise when children or adolescents are involved. For example, the DSM-IV TR criteria for bipolar and manic disorder were originally developed for adults but right now, after a few changes, it is used for children as well. This increases the chance of misdiagnosing children with bipolar disorder. This information is supported by the fact that in the last 10 years, there has been a 40%4 increase in the number of children diagnosed with bipolar disorder. It is believed that there is not a sharp increase in the number of children with bipolar disorder but rather that the clinicians have been applying the diagnostic criteria (which were originally developed for adults) much more aggressively to children.5This is one of the weakness of DSM-IV TR that needs to be improved in the future as the effects could be devastating. One of the weakness, which I believe is not very significant but still is a problem and which is actually questioned by people is the definition of the mental disorder. This was also mentioned in the lectures. According to DSM-IV TR, a mental disorder is â€Å"associated with present distress or disability or significant increased risk of death, pain, disability and important loss of freedom†. This raises the question.. what about those  individuals who engage in activities that threatens their lives but they actually enjoy it? Such as mountain climbers, scuba divers, etc. Obviously they are no considered as abnormal but according to the definition of the DSM, they are considered as having a mental disorder. But we know that this is not correct. Thus, there is a problem with the definition of the word ‘mental disorder’. There are many other weaknesses as well such as with Personality Disorder diagnosis. It is believed that the description of symptoms is very broad. This means that patients diagnosed with the same disorder could actually have very different clinical presentations. For personality disorders, DSM uses a categorical approach6. However, it would be better to use a dimensional approach so that the different types/ degree of disorder could be distinguished and thus, could be given different and more efficient treatments. Another major weakness of the Axis II personality disorder is that there is a very high degree of overlapping or co-occurence with each other. This is also a problem for Axis I mental disorders.7 There are many other weaknesses with specific disorders of DSM especially about the diagnosis criteria and some other issues such as whether ‘Paraphilias’ should be included or not. Most of the major ones has been discussed above. Now, the new changes for DSM-V will be discussed. There are many new changes for the DSM-V. I will mainly mention those that are related to the weaknesses mentioned above. However, the new changes are discussed, the 4 principles behind the current process for revising DSM should be discussed. (Obtained from APA DSM-V Development’s official website) 1) Clinical Utility- the manual should be useful to those who diagnose and treat patients with mental illnesses 2) Recommendations should be guided by evidence 3) DSM-V should maintain continuity with previous editions whenever possible 4) No priori restraints should be placed on the level of change permitted  between DSM-IV and DSM-V From these 4 principles, it becomes clearer to us why DSM-IV is being revised and what changes should be expected. One of the changes is that in order to better assess the severity of symptoms, a dimensional assessment will be included. For example, other factors such as sleep quality, mood, etc will be considered regardless of the diagnosis. This will help to reduce the problem of â€Å"mechanical approach† as mentioned before. They will also help to address symptoms that are not included within the diagnostic criteria for specific illnesses (e.g. the problem of insomnia for patients diagnosed with Schizophrenia). DSM-V might also include something that could solve the problem of how to handle patients with co-occuring disorders, which is a major weakness of DSM-IV TR. Besides that, DSM-V will also have improved diagnostic criteria that are not precise in DSM-IV TR. Also, a few words would be changed as well in the new DSM. For example, the word â€Å"Mental Retardation† would be changed to â€Å"Intellectual Disability†.   Another change that would also be included would somehow, indirectly address the problem of cultural bias. In the new DSM, careful consideration would be given to the gender, race and ethnicity. This, I believe could help to lessen the severity of the cultural problem as mentioned before. Besides that, another ‘proposal’ is that, the criteria should be more stringent for the diagnosis of bipolar disorder in children. This also addresses one of the weakness mentioned earlier. This is a very important proposal as I believe it is not very humane to misdiagnose children with â€Å"Mental Disorders†. It could affect their life significantly. Another major changes is in the assessment and diagnosis of personality disorders. As mentioned before, a dimensional approach will be used rather than a categorical approach. This can help to determine the different severities of disorders and determine the ‘subtypes’ more accurately. Overall, it would make the diagnosis much more accurate and thus, more  efficient treatment could be used. These changes were mainly in response to the weaknesses mentioned above. Other changes include creating a new category called â€Å"Behavioural addiction†. This category is solely based on gambling. Internet addiction was also proposed. However, due to the lack of research in that field, it was rejected. This shows that they somehow followed principle 2 which emphasizes on the importance of evidence. Another category called the â€Å"Risk Syndromes† is also being considered. This will help clinicians identify earlier stage of mental disorders. The category of substance abuse and dependence will be removed and it would be replaced by a new category called â€Å"Addiction and Related Disorders†. This is done to help clinicians distinguish between similar types of disorders more easily and lower the chance of misdiagnosis (e.g. dependence is often confused with compulsive drug-seeking behaviour addiction). 8Another change is that the criteria for some eating disorde rs such as Anorexia Nervosa has been improved. Overall, the diagnostic criteria for many disorders has been improved and a dimensional approach is being used in DSM-V rather than the categorical approach used in the previous editions. All of these changes, together with many other minor ones, aim to improve the diagnosis process and thus, ensures that people are saved from misdiagnosis (which could lead to devastating effects) and so that they can receive better treatment which would increase their chances of being cured (if the disorder is curable). Guy De Maupassant’s The Jewelry Essay

According to critic W.E. Garrett Fisher, â€Å"(Maupassant’s) view of human nature was an utterly distorted one. On all hands he only saw the cruelty, the bestiality, above all, the ineffable stupidity of mankind. We hardly find one man or woman in his books who illustrates the nobler side of life.† (Piana, n. pag.)   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   Indeed, French writer Guy De Maupassant’s works were based on ordinary incidents from everyday life, which exposed the true nature of people. (n. pag.) The Jewelry was no exception – it was built on the premise that first impressions of people are almost always wrong, as they constantly change, with unexpected results.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   Set in Paris, The Jewelry centered on Monsieur Lantin, a chief clerk in the Department of the Interior, and his wife, who was unnamed throughout the story. The initial impression that the reader can derive from Madame Lantin is that she is a perfect woman. De Maupassant described her as â€Å"a perfect type of the virtuous woman in whose hands every sensible young man dreams of one day intrusting his happiness.† (n. pag.) The reader can also conclude that Madame Lantin was the ideal wife and homemaker. According to De Maupassant, â€Å"She (Madame Lantin) ruled his home with an economy so adroit that they really seemed to live in luxury† and that â€Å"It would be impossible to conceive any attentions, tendernesses, playful caresses which she (Madame Lantin) did not lavish upon her husband†¦Ã¢â‚¬  (Piana, n. pag.) De Maupassant implied through the latter that Madame Lantin was a faithful spouse who will never cheat on her husband. (Piana, n. pag.)   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   However, The Jewels’ plot identified Madame Lantin as the antagonist by unmasking the hypocrisy of her character. After her demise, Monsieur Lantin wondered how did she manage to make ends meet with his paltry salary of 3,500 francs a year. Madame Lantin had very expensive vices – frequenting the theater and her large collection of fake jewelry. De Maupassant wrote, â€Å"His salary, which, in his wife’s hands, had amply sufficed for all household needs, now proved scarcely sufficient to supply his own few wants. And he asked in astonishment how she had managed always to furnish him with excellent wines and with delicate eating which he could not now afford at all with his scanty means.† (Piana, n. pag.)   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   It was also The Jewels’ plot that answered Monsieur Lantin’s question. Broke and hungry, he was forced to go to the jewelry store in an attempt to pawn Madam Lantin’s jewels – and came out of the shop with 196,000 francs. Therefore, this incident revealed that Madame Lantin’s jewels were genuine, in sharp contrast to her claim to her husband, â€Å"Now look at them – see how well the work is done. You would swear it was real jewelry!† (Piana, n. pag.) It also refuted Madame Lantin’s fidelity – she most likely had a paramour who provided her with money and jewelry. (Piana, n. pag.) After Monsieur Lantin told the jeweler that he had more jewels to sell to him (he managed to sell one of his wife’s necklaces for 18,000 francs), â€Å"one of the jewelry store’s clerks rushed out to laugh at his ease while another kept blowing his nose as hard as he could,† as if they were trying to suppress their laughter. (n. pag.) Chances are, they were mocking his ignorance about his wife’s adultery. The story ended six months later with Monsieur Lantin’s unhappy second marriage to a virtuous but ill-tempered woman. These circumstances allowed him to emerge as the protagonist – the loving husband who was cheated on by his late first wife and was trapped in a loveless second marriage.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   The Jewelry was able to defend its argument that looks can really be deceiving. Madame Lantin, whom everyone praised and admired, turned out to be a greedy woman who cuckolded her husband for material wealth. Meanwhile, Monsieur Lantin became as fortune-hungry as she was by enjoying the money he got from selling her ill-gotten jewelries. Money transformed him from an upright man who refused to sell a fake piece of jewelry to a man obsessed with profiting from his late wife’s jewels, regardless of the fact that she got these using illicit means. What added credibility to The Jewelry was that De Maupassant allowed the events in the story to naturally unfold and expose the characters’ hypocrisy in the process, drawing on the saying that there is no secret that will never be revealed. It was written and directed by Franco Zeffireli and starred Olivia Hussey and Leonard Whiting. Released in 1968 the film was the most financially successful film made up to that time because of the young teenage audience it had attracted. Although it may not be appealing to the teenagers in this century. It followed the Original storyline very clearly and the costumes were very much like those it the time it was made. Baz Luherman;s version of Romeo + Juliet and starring Leonardo Di Caprio and Claire Danes was a modernization of William Shakespeare’s play. This was a very successful version of the story and many viewers around the world loved the great difference in the interpretation. Act 1 Scene 5 of Romeo and Juliet is a very essential part in understanding the tragedy of the story. It is where they first meet and fall in love at the Capulet’s masquerade ball. Juliet being a Capulet and Romeo being a Montague creates confusion and disbelief for the audience. In my opinion the film that portrays this in a better way is Luhermans version. Compared to Zeferellhis traditional movie it keeps the audience interested in what is happening. Some things that contribute to this is the mi –sen –scene music, lighting, camera angles and costumes. The music in Zeherellis movie is very traditional, when Romeo and Juliet meet eyes for the first time in the movie the music becomes soft and sweet, the typical love-song music of Shakespeares time. The setting is at Capulets masquarde ball and in an old castle made from stone and has candle lit lighting. Just like the music, lighting and location the costumes stay very true to the Elizabethan era. There is not much of a variety in the camera angles and it usually focuses on the actors facial expression. This movie would have been more of what Shakespeare intended. Luhermans version on the other hand has a very different atmosphere. This modernized adaptation has much more up tempo and disco music. Although when Romeo and Juliet first meet the slow song ‘Kissing you’ is playing in the background adding an addition of romance to the scene. The lyrics reflect their relationship and is ended with the kissing. The setting is very modern and instead of a masquerade ball there is a costume party. The start of this scene is very upbeat and lively. The lighting is much brighter than Zeherelli’s version and at the beginning there is flashing lights. Luherman was more experimental with his camera angles using medium shots, long shots, tilted angles, and spinning effects making the audience feel more involved in the scene. These two movies have given more of a great depth to the tragic love story of Romeo and Juliet without taking away the beauty of the language. Most animal abuse goes unreported and nothing has been done to fix the problem of owners locking their pet up and leaving them for long periods of time without food, water, or care. The most common form of animal abuse has to do with an owner locking their dog or pet up for times on end, just completely neglecting them and not giving them any care or attention. More than half the cases involving animal cruelty had to do with pets. There are little laws about animal abuse or animal cruelty. In some states its actually taken seriously like in Colorado abusing or putting an animal through any kind of cruelty is a $500,000 fine, up to 6 years in jail, and a ban on owning or being around any form of living animal. If the government would follow through on paying attention to the case of animals being abused or treated unfair itself, there would be harsher punishment for an act as of doing so. Giving animals the rights they deserve is how it should be instead of using animals as lab rats, test subjects, or a teenagers thought of just a prank, when really in realization pushing a cow over on its back as just a prank can be very deadly for the cow actually. Animals arent given that much of their rights as they have to begin with. If they were to investigate and go deeper into the topic and subject of animals being abused there would be a lot of things that they would find out that they didnt know. Harsher punishment is another thing to talk about. If they had harsher punishment just for neglecting an animal and not taking care of it there would be a lot more animals safe. If there was harsher punishment for abusing an animal there wouldnt be as many animals getting abused, beaten, or neglected. Overall, I was amazed at the use of color, defined lines, and emotion that this artist conveyed in his painting. The wall adjacent to me was full of photographs; some were full color, some in sepia, and others black and white. I glanced at this middle-aged woman, dripping in pearls and cashmere, who had one hand on her heart, and the other held her complimentary champagne close to her body as she stared at this one photograph, a black and white photo of a single muddy footprint. I was astounded at how in awe she appeared to be, almost as if she could burst into tears at any moment. I had to know what she saw in this photograph that had her so awestricken.

It is called the corrida de toros in Spanish; the bullfight takes place in a large outdoor arena known as the plaza de toros. The object is for one of the bullfighters (toreros) the matador to kill a wild bull, or toro, with a sword. At best, the term â€Å"bullfighting† is a misnomer, a confused, maimed, psychologically tormented, and physically debilitated bull. Supporters justify the act by calling it a tradition.Opponents maintain that no matter what its history, bullfighting is the torture, mutilation, and slaughter of animals for entertainment. Bullfighting can be traced back to ancient days. They were popular spectacles in ancient Rome, but it was in the Iberian Peninsula that these contests were fully developed by the Moors from North Africa who overran Andalucia in AD 711. Bullfighting developed into a ritualistic occasion observed in connection with feast days, on which the conquering Moors, mounted on highly trained horses, confronted and killed the bulls.The Minoans of Bronze Age Crete practiced bull leaping as part of religious ritual, and later Greek and Romans also had rites that involved the slaughter of bulls. The Moors, who fought bulls from their horses and killed them with javelins, probably introduced the sport to Spain (c. 11th cent. ). The bullfighter has to demonstrate his skills to the audience in a dramatic and enthusiastic way. If he makes a good job of it and gets a good round of applause at the end, he could be bestowed the ultimate honour by being presented with one of the bull's ears or its tail.If he has been really good he may even be carried round the arena. One variation of the traditional bullfight is bull running where bulls are released into the streets and masses of participants do their best to avoid them and stay alive. Pamplona is the best place to see this. One of the oldest bullrings in Spain is loca ted in the hilltop village of Ronda in Andalucia, southern Spain. It is the venue for one of the most spectacular corridas of the year. In early September the ring hosts a week-end of bullfights in celebration of Pedro Romero, the Saturday corrida being staged in Goyesc costume for which tickets are very hard to obtain.The bullring in Ronda is open to the public, with a small admission charge. The bulls were mostly moribund.â⠂¬ (2)A study conducted by scientists at Spain’s Salamanca University found that 20 percent of the bulls used for fighting are drugged before they step into the ring. In the sampling of 200 bulls, one in five had been given anti-inflammatory drugs, which mask injuries that could sap the animal’s strength. One common practice is to â€Å"shave† the bulls’ horns by sawing off a few inches. (3) Bulls’ horns, like cats’ whiskers, help the animals navigate, so a sudden change impairs their coordination. Shaving is illegal, so a veterinarian sometimes inspects the horns after a fight.But in 1997, the Confederation of Bullfighting Professionals, including Spain’s 230 matadors, went on strike in opposition to these veterinary inspections. The bulls aren’t the only victims of the arena. The horses used in bullfights are blindfolded so that they don’t become frightened of the charging bull. They are often gored. At the Barcelona Olympics, The Philadelphia Daily News reported, â€Å"One time last night, the bull charged the horse, knocked off the rider, knocked over the horse and got his horn tangled up in all the padding.The frightened horse, still blindfolded, kicked furiously as it lay on its side. Eight men, known as â€Å"grabbers† line up in front of the bulls to grab the bull’s horns, piling on top of the animals and tackling them until they fall to the ground. (6) In Portuguese bullfights in many countries, the bulls are still stabbed with banderillas and have their horns shaved. (7)Although tormenting and abuse is part of the show, killing must be done outside the arena. In Colombia, there is an annual festival in which solitary bulls are tormented by thousands of people who think they are testing their â€Å"bravery† (aided by a festive atmosphere and large quantities of alcohol). â€Å"If nobody gets killed, it’s boring,† laments Carlos Perez, head of the committee that organized the contest in 1996. But even Colombian bullfighter Luis Cuadrado admits, â€Å"It’s just one bull against a thousand. morons.†Cuadrado prefers to sit on the ground until the bull is close enough to stab with a lance, after which Cuadrado promptly scurries away to safety. These festivals last four or five days, with at least 35 victimized bulls each day.
